The Hospital Accident Insurance Plan benefits described are underwritten by Zurich American Insurance Company (Zurich), 1299 Zurich Way, Schaumburg, IL 60196,1-800-987-3373 (NAIC #16535). This insurance provides limited benefits and is intended tohelp supplement Comprehensive Coverage. See the site for terms and conditions, including exclusions and limitations. This insurance does not provide Major Medical or Comprehensive Medical Coverage and is not designed to replace Major Medical insurance. Coverage may not be available in all states or certain terms, conditions, and exclusions may be different where required by law.
2. Accidental death and dismemberment insurance is underwritten by Minnesota Life Insurance Company and is available for members 18 or older. Additional coverage can be purchased at group rates to include up to $250,000'. All coverage reduces by 50% at age 70 and older, regardless of age at enrollment.
